Good Shepherd Rehabilitation’s Outpatient Patient Portal provides convenient access to your medical information.

• Once you schedule an Outpatient Rehabilitation evaluation with us, you will receive a Patient Portal Welcome email containing your username, initial password and link to the portal (via Raintree).
• If you did not receive the email or have questions, email patientportal@gsrh.org for assistance or contact your therapy clinic directly.

Athena Portal (Doctors and Psychologists)

With Athena, you can access your personal healthcare information for all Good Shepherd Rehabilitation doctor and psychology services.
