A 99-bed facility, Good Shepherd Home at Conrad W. Raker Center and is open to residents 12-years-old or older with primary diagnoses including, but not limited to, cerebral palsy, muscular dystrophy, quadriplegia or Multiple Sclerosis.

Residents may be referred by a physician, insurance company, discharge planner/social worker, home health agency, family member or self-referral.
