Pain Rehabilitation: Program Admission

Pain Rehabilitation Program Admission Criteria

1. Diagnosis of a chronic (three- to six-month month duration or longer) pain disorder (fibromyalgia, back pain, neuropathic pain, etc.)
2. Primary pain disorder is not limited to a headache, dental, abdominal, gastrointestinal, genitourinary, or gynecologic problem.
3. Patient is at least 18 years of age.
4. Patient has been referred by a physician, chiropractor, or other appropriate healthcare provider.
5. Patient is medically and psychiatrically stable.
6. Patient does not have an active addiction disorder.
7. Significant rehabilitation goals are indicated for patient. Examples:

  • Management of pain via pharmacologic and cognitive-behavioral interventions
  • Improved strength/conditioning to compensate for functional limitations, decreased mobility, proneness to fatigue, etc.
  • Development of daily living and self-care skills to support improved independence of functioning
  • Facilitating patient and family adjustment and coping via education and aftercare planning
  • Facilitating patient involvement with available vocational rehabilitation resources (OVR, etc.)

8. Patient is able and willing to collaborate with the rehabilitation team in goal setting and is accepting of a self-management approach to his or her pain disorder.
9. Intensity and frequency of any other treatment that the patient may be undergoing concurrently does not conflict with the patient's ability to participate in pain rehabilitation program.
10. Patient and the referring/primary treating healthcare provider plan to continue follow-up during and after the program.
