Admission Information
- Applicants may refer themselves to Good Shepherd Home—Allentown or Good Shepherd Home—Bethlehem or be referred by family members, agency representatives, pastors, physicians, or other healthcare providers.
- Applicants must be age 12 or older.
- Applicants must have a primary diagnosis of some form of physical disability including, but not limited to, cerebral palsy, spina bifida, Multiple Sclerosis, or quadriplegia.
- Applicants must be determined to require the level of care provided by a nursing facility and have affirmative determination of Nursing Home Service Eligibility by the designated evaluating agency.
- Applicants meeting the above criteria may request that an application be faxed or mailed to them. Requests and/or questions may be directed to:

Application Process
- Upon receiving application packet, applicant or his or her representative must complete the application form and financial form in full. Applicant must also sign and date the five "release of information" forms.
- Applicant must have a physician complete and sign the medical evaluation form.
- Applicant must obtain and submit the required financial documents outlined on the financial form.
- Applicant must return the completed application form; financial form; medical evaluation for and signed "release of information" forms; a recent photograph; any available medical, social, and psychological reports; and required financial documents to:

- Good Shepherd will obtain additional medical and psychosocial information, as needed, and as indicated by the application.
- All resident data will be reviewed by the admissions committee.
- If found to be an eligible candidate, applicant will be invited for a tour and interview conducted by the admissions committee.
- Final decision regarding eligibility will be made by the admissions committee following the tour and interview. Written notification will be provided to the applicant.
Approved applicants will be placed in a waiting pool and be kept informed of bed availability. Prior to admission, each applicant must be pre-screened for nursing home admission. Additional forms may need to be completed by the applicant's family physician and returned to Good Shepherd. These forms will be sent to the designated evaluating agency in Pennsylvania.
After applicant has been evaluated by the agency, he or she will receive written notification of his or her eligibility or ineligibility for nursing facility services. Out-of-state applicants will be reviewed by the designated agency to determine service eligibility.
NO APPLICANT CAN BE ADMITTED WITHOUT AN AFFIRMATIVE DETERMINATION FOR NURSING FACILITY SERVICES BY THE DESIGNATED EVALUATING AGENCY.
