Aging in Place: Designs for Life Home Modification Program

Home Modification

Good Shepherd is committed to helping you remain independent and to age comfortably in your own home.

Through the Designs for Life Home Modification Program, an occupational therapist can visit your home and develop a design plan with the goal of helping you function safely and independently in your home.  The plan is structured to meet your needs in the most aesthetically pleasing manner possible possible.

If you are building a new home, an occupational therapist can meet with you and your contractor to review the design plans prior to construction.  We will make recommendations to ensure that your home is built in a manner that allows you to live safely and independently.

Elements of an Aging in Place Home Modification Plan may include:

  • Recommendations to increase home safety and prevent falls
  • Pre-construction consultation
  • Remodeling and renovation projects
  • Lifespan design
  • Assessments regarding home modification and the appropriate use of adaptive equipment
  • Plans to make your home universally accessible
Home Modification

For each home safety/home modification evaluation, an occupational therapist will visit your home, interview you and perform a functional, cognitive and environmental assessment. The therapist will complete a report with recommendations to increase your safety and mobility inside your home.

Once you are provided with a safety/home modification plan, you can choose to implement all or some of the recommendations. The work can be done by whatever contractor you choose. If requested, our specialist will provide information regarding funding sources and contractors.

A typical consultation takes approximately one hour.  Call 610-776-3295 to inquire about rates.
