Long-term Care: Admissions ProcessIf you or a loved one would like to apply for admission to either the Conrad W. Raker Center in Allentown or Good Shepherd Home-Bethlehem, please adhere to the following criteria and admissions process. Criteria for admission:

- Completed application form Upon receipt of completed application and required documents, Good Shepherd will obtain additional medical and psychosocial information as needed and indicated by the application. All data will then be reviewed by the admissions committee. If found to be an eligible candidate for admission, the applicant will be invited for a tour and interview conducted by the admissions committee. A final decision regarding eligibility then will be made by the admissions committee. The applicant will receive written notification of the committee’s decision. Approved applicants will be placed in the waiting pool and be kept informed of bed availability. Periodically, applicants may receive requests for status reports and be asked to update admission preference and intentions. NOTE: No applicant can be admitted without first receiving approval for nursing facility services by the designated state/county agency. The admissions coordinator will provide direction and assistance with this process.
